The PatientтАЩs Critical Condition and Surgical Challenge

The 62-year-old patient was suffering from Bicuspid Pinhole Severe Aortic Stenosis, a serious congenital heart condition where the aortic valve has only two flaps instead of the normal three, causing the valve to narrow severely and restrict blood flow from the heart to the body. Compounding this, the patient had multiple co-morbidities that ruled out traditional open-heart surgery.

The Unique TAVR Route

Normally, a TAVR procedure involves guiding a wire (acting as a track) from the groin artery backward across the blocked aortic valve into the left heart. However, in this case, the patient’s native valve was so severely blocked, deformed, and calcified that the wire could not cross it via the standard route.

Dr. JaswalтАЩs team innovated the procedure, opting for an alternative route that required navigating the complex cardiac structure:

  1. Groin Vein to Right Heart: Accessing the heart via the groin vein to the right chambers.
  2. Septal Puncture: Crossing over through a septal puncture (a small hole created in the wall separating the heart chambers) into the left heart.
  3. Opposite Direction: Crossing the diseased valve in an opposite direction aided by the natural flow (jet) of blood.

During the successful operation, a new aortic valve was precisely placed inside the damaged, narrowed one. The new valve expanded, displacing the old valve and immediately restoring normal blood flow.

The patient experienced a smooth post-operative recovery and was discharged a mere three days after the procedure, highlighting the significant benefits of this minimally-invasive technology.
